Zachary D. Rowitsch 6d1c776f8e Add persistent investigation reports with fix tracking ...

Move investigate skill output from /tmp to investigations/<site-stem>/
with gitignored artifacts/ and tracked report.md. Reports use stable
finding IDs (F-001, F-002, ...) with Open/Fixed status tracking, so
re-investigating the same site preserves history and marks resolved
issues with the date they were confirmed fixed.
